Our trained and passionate care staff provide a wide range of flexible, high-quality home care services to adults across Pontefract and surrounding areas, ensuring that every client experiences a sense of hope, comfort, and connection in their home environment.
Our CQC-Approved Services
All regulated services are provided under our CQC registration and are designed to meet the individual needs of adults requiring support in their own homes.
Personal Care and Support
Our personal care service ensures that adults receive assistance with all activities of daily living, tailored to their specific needs. This includes:
– support with washing, bathing, dressing, grooming, and oral care, skin care and personal hygiene. Our Health Care Assistance also help with continence management, meal preparation, diet monitoring, and assistance with eating and drinking. Medication administration or prompting is provided to ensure clients take their prescribed medicines safely. All care is delivered with respect for the client’s independence, dignity, and personal preferences.
Supported Living (Live-In Care)
Our supported living service provides 24/7 care for adults who require continuous assistance while remaining in their own homes. Staff may sleep over or stay awake overnight, depending on the individual’s needs. We support personal care, wellbeing, and daily activities such as managing bills, cooking, and shopping. Additionally, we assist clients in accessing education, employment, community activities, and social opportunities. Our goal is to promote independence while ensuring a safe and supportive environment.
Overnight Care and Live in Care
We offer two types of overnight care to meet individual needs. The sleepover option allows staff to reside in the home overnight, providing care and monitoring as required. Waking night support is provided when staff are awake throughout the night, actively assisting clients and ensuring their safety.
Live-In Care
Live-in care provides continuous support while allowing individuals to remain in the comfort of their own home. A trained carer stays in the home to provide personalised assistance throughout the day and night, ensuring safety, companionship, and support with daily living.
Hospital Discharge Support
Our hospital discharge service ensures a safe and smooth transition from hospital to home. We provide short-term intensive support tailored to each client’s recovery needs. This includes personal care, medication prompts, meal preparation, mobility assistance, and monitoring health and wellbeing. We collaborate closely with hospital staff, social workers, and community healthcare providers to create personalised care plans that facilitate recovery and prevent readmission.
Companionship Care
Companionship care is designed to reduce social isolation and promote emotional wellbeing. Clients are matched with companions based on their interests and personalities. Companions engage clients in meaningful activities such as social outings, hobbies, meal sharing, or attending events. This service ensures that clients remain socially connected, supported, and engaged, enhancing their quality of life and overall wellbeing.
Respite Care
We understand that family carers sometimes need time to rest and recharge.
Our respite care services provide temporary support, ensuring that loved ones continue to receive high-quality care while family members take a break.
Dementia Care
We provide compassionate care for individuals living with dementia and memory-related conditions. Our support centres on the personhood of the individual supported – their values, culture, beliefs and what matters most to them. We realise this by working closely with the individual, their family and the people that matter to the individuals we support, ensuring that the individual’s priorities and personal preferences are at the core of service planning and delivery.
Our carers are trained to provide reassurance, routine, and structured support that helps people remain comfortable and safe in familiar surroundings.
Support may include: Daily personal care, Memory prompts and reassurance, Supervision and safety support, Assistance with daily routines, Emotional support for individuals and families, support to engage in activities or access the community.
Mental Health Support
We provide supportive care for individuals experiencing mental health challenges, helping them maintain stability, independence, and daily routines.
Our care focuses on promoting wellbeing, confidence, and engagement with everyday life.
Support may include:
Assistance with daily living tasks, Encouragement with routines and self-care, Companionship and emotional support, Support to access community services and appointments
Physical Disability Support
We support adults living with physical disabilities to maintain independence and continue living safely at home.
Our carers assist with mobility, personal care, and daily tasks while promoting choice and independence.
Medication Support
Our trained care staff can assist with medication management in line with care plans and professional guidance.
Support may include:
Medication reminders, Prompting medication, Administration of medication where appropriate, Assistance collecting prescriptions
Meal Preparation and Nutrition Support
Maintaining good nutrition is essential for health and wellbeing.
Our carers can assist with:
Preparing nutritious meals, Supporting individuals to eat and drink, Encouraging healthy dietary habits, Monitoring food intake where required
Our Other Services
These additional support services do not require CQC registration but complement our regulated services, enhancing clients’ independence and wellbeing.
Housing Support and Instrumental Activities of Daily Living
We assist clients with tasks that help them maintain their homes and daily routines, ensuring a safe, habitable, and comfortable environment. This includes weekly shopping, light and heavy household chores, laundry, managing correspondence, and financial management. By maintaining a clean, organised, and functional home, we help clients live independently and with confidence.
Day Centre Support and Community Engagement
We provide assistance for clients attending local day centres, social clubs, or educational programs. This includes transport, supervision, and practical support to ensure safe participation. By attending day centres, clients remain socially active, engaged in meaningful activities, and connected with their communities.
Social and Community Engagement Support
Our team supports clients in accessing wider community activities, attending local events, volunteering opportunities, or educational programs. This helps clients maintain active lifestyles, develop skills, and maintain meaningful social connections.
Administrative and Practical Assistance
We assist clients with tasks such as organising schedules, attending appointments, liaising with external agencies, or managing correspondence. This practical support ensures clients can manage day-to-day responsibilities confidently.
